We accept Cash, Credit, Personal Checks or Insurance.

 We are an out of Network provider and in order for us to file with your insurance company, a prescription from your doctor with diagnostic codes is required.   Also a front and back copy of your insurance card is required.
We Do Not accept Medicare insurance for blood work.  

Healthchek bills standard insurance prices to your insurance carrier and not the discounted prices we have listed. If your insurance does not pay, you are only responsible for the discount price plus a $15 filing fee.
