We are dedicated to providing affordable wellness screenings and

other low-cost medical testing. Our main office is located in

Northwest Indiana and we have been servicing the greater Chicago

land area for over 20 years.

We offer services in our office or at many monthly on-site

If you are interested in scheduling a screening event for your

company, church, school etc. please feel free to contact us.

We can customize our services to fit your needs.

Important Payment Information

We accept Cash, Credit or Check. We also file insurance and we are an out of Network provider. A front and back copy of your insurance card is required. We Do Not accept Medicare insurance.

Screenings are billed as routine blood work unless accompanied by a doctor's order that includes diagnostic codes.

Healthchek bills standard insurance prices to your insurance carrier and not the discounted prices we have listed. If your insurance does not pay, you are only responsible for the discount price plus a $15 filing fee.
